The history of the Church of the Assumption of the Blessed Virgin Mary with Sanctuary of Our Lady of Dzików and the adjacent Dominican monastery is related to the miraculous painting of Our Lady of Dzików. The three-nave, Baroque church consists of, among others, the former altar of the chapel in Dzików Castle, and the tombstone of Juliusz Tarnowski. The church's crypt is a place of rest of family members of the building's founders. The monastic cemetery is located nearby.

The church is situated in the place of the wooden church and monastery complex from 1677 which burnt in 1703. The church and monastery were founded and donated to the Dominican Order by the Tarnowski Family. The church was destroyed during World War I.

The church and monastery were built in order to protect the painting of Our Lady of Dzików. It was moved from place to place many times (one of the places it was housed was the Tarnowski Palace). Today, it can be admired at the main altar in the sanctuary.
